Diagnostic Assessment
The DA establishes an initial mental health diagnosis and determines whether ARMHS services are medically necessary. It includes an evaluation, conducted in person or virtually of an individual’s symptoms, functional impairments, distress, strengths, and available resources.
Functional Assessment
The FA identifies how a person’s mental health symptoms affect specific areas of daily life. It highlights both strengths and areas of impairment in domains such as social interaction, self-care, independent living, and financial management. The FA helps confirm eligibility for ARMHS by showing the connection between symptoms and functional needs.
Individual Treatment Plan
The ITP is a collaborative plan developed between the client and provider. It outlines personalized goals, specific skill-building objectives, and the services or interventions that will support recovery. The ITP serves as the guiding document for the delivery and monitoring of ARMHS services.
